DELIVERY POLICY

• Purpose of this Policy
|
• This policy is set in place to specify standard procedures
for prescription deliveries. This policy can be changed at any time without notice.
Any inquiries should be made to the Pharmacy Owner or Pharmacy Manager. • Delivery
Procedures
|
• “AME Pharmacy, LLC” provides its delivery service to patients
within the defined delivery range. All prescriptions must be verified by the Pharmacist
in Charge prior to leaving the pharmacy. “AME Pharmacy, LLC” only provides delivery
of prescription medication, and each delivery must meet the minimum required number
of prescriptions as defined by the “AME Pharmacy, LLC” management. “AME Pharmacy,
LLC” reserves the right to deny delivery to any customer for any reason as decided
by management. Management also reserves the right to restrict the number of deliveries
a specific customer can receive. The delivery times are defined as “AM” and “PM.”
“AM” deliveries are typically between 10am and 2pm and “PM” deliveries are typically
between 2pm and 6pm. Delivery is offered Monday through Friday and is not offered
on weekends or holidays.

• Delivery Minimum
|
• “AME Pharmacy, LLC” staff reserves the right to require
a minimum number of prescription in order to offer free delivery. Deliveries that
do not meet the defined minimum may require the charging of a delivery fee.

• Delivery Range
|
• Though subject to change, “AME Pharmacy, LLC” defines
its delivery radius as 20 miles from the address of 731 Frederick Rd. The pharmacy
may also ship medications as agreed upon with the patient and pharmacy staff.

• Delivery Fee
|
• A delivery fee may be charged to patients as decided by
“AME Pharmacy, LLC” management on a case-by-case basis. All customers are responsible
for paying the delivery fee upon delivery. All insurance co-pays and delivery fees
are final. For questions concerning delivery fees, please contact “AME Pharmacy,
LLC” management. For questions concerning insurance co-pays, please contact the
patient’s insurance company.

• Accepting of Delivery
|
• All prescription deliveries must be signed for
by the receiving party. Once the prescription is signed for, “AME Pharmacy, LLC”
is no longer liable for the listed prescriptions. The customer may designate a representative
to sign for him/her.

• In the event that no one is able to sign for the prescription
delivery, the customer may authorize an “AME Pharmacy, LLC” licensed driver to leave
the prescription in a specified area. In this situation, a member of the “AME Pharmacy,
LLC” staff may write “DELIVERED” on the delivery ticket in place of the receiving
party’s signature. If the customer authorizes an “AME Pharmacy, LLC” licensed driver
to leave the prescription(s), the customer thereby assumes all responsibility for
all prescriptions and items left.

• Shipping of Prescriptions
|
• “AME Pharmacy, LLC” may ship prescription medication through
UPS, FedEx, USPS, or other carriers for reasons agreed upon with the customer and
pharmacy staff. All shipments must be verified by the Pharmacist in Charge prior
to leaving the pharmacy.

• Reporting Concerns
|
• Any concerns about an “AME Pharmacy, LLC” licensed driver
or other employee can be directed to the Pharmacy Manager.
